Position Overview
We are seeking a compassionate Licensed Vocational Nurse (LVN) to provide skilled nursing care to patients in the field. In this role, you will follow a physician-approved care plan while collaborating closely with the Case Manager/RN and interdisciplinary team. Key Qualifications
Active California LVN license from the California Board of Vocational Nursing and Psychiatric Technicians.
1+ year of nursing experience (community/home health or medical/surgical preferred).
CPR certification (current and valid).
Strong clinical judgment with knowledge of current nursing practices.
Effective verbal and written communication skills.
Must pass a pre-employment physical (including Mantoux TB test or chest X-ray).
Able to walk, bend, stoop, and lift objects as needed.
Licensed driver with insured vehicle (non-drivers must sign an Auto Release of Liability).
Fluent in English, with excellent organizational skills and the ability to work independently.
Familiarity with HIPAA, mandated reporting, and healthcare regulations (e.g., Title 22).
Primary Responsibilities
Develop and implement nursing care plans in consultation with the interdisciplinary team.
Provide safe and effective care in accordance with the LVN Practice Act.
Educate patients, families, and caregivers about care plans, tests, and health conditions.
Maintain accurate and timely documentation, ensuring compliance with agency policies and third-party requirements.
Monitor and report any significant changes in the patients condition to the physician or RN.
Participate in home health service coordination, collaborating with the Case Manager or Clinical Supervisor as needed.
Attend case conferences, staff meetings, and in-service training sessions.
Follow proper infection control procedures and adhere to Standard Precautions.
Support the RN and physician with specialized procedures, ensuring aseptic techniques are followed.
Arrive on time for assigned visits/shifts and report cancellations promptly.
Maintain up-to-date credentials and stay informed about healthcare regulations.
Report incidents, fraud, abuse, or patient safety concerns as required.
Physical Requirements
Frequent walking, bending, stooping, and lifting as required for patient care.
Use of hands to operate medical equipment and technology.
Must be free from illness and infections to protect patients and staff.
