Job Opening: ECM Case Manager

Location: On-site (Field Work)

Pay Range: $24 Per Hour

Reports To: Director of ECM

Position Overview

We are seeking a dedicated ECM Case Manager to coordinate clinical care for clients in the Enhanced Care Management (ECM) Program. This role focuses on helping clients access healthcare, housing, and community resources while ensuring continuity of care. Most work will take place in the field, building strong relationships with clients and providers to promote long-term stability and independence.

Key Qualifications

• 2+ years working with homeless, low-income, or high-risk populations.

• Bachelor’s degree in healthcare or related field preferred.

• Knowledge of housing resources and government benefits systems.

• Strong clinical judgment and communication skills.

• Licensed driver with insured vehicle.

• Current CPR certification.

• Pre-employment physical and TB screening required.

Primary Responsibilities

• Coordinate and monitor care plans for ECM clients.

• Build relationships with housing providers, healthcare professionals, and community partners.

• Facilitate access to mental health, substance abuse, and medical services.

• Use trauma-informed care, motivational interviewing, and harm-reduction techniques.

• Monitor client progress, treatment adherence, and service delivery.

• Maintain accurate records and ensure compliance with agency policies.

• Participate in quality improvement and training activities.
