ECM Outreach Coordinator (Riverside)

JOB DESCRIPTION

  • Position: Enhanced Care Management (ECM) Outreach Coordinator
  • Pay Range: $24-$28
  • Reporting To: ECM Manager
  • Work Type: Hybrid

POSITION SUMMARY

The Enhanced Care Management (ECM) Outreach Coordinator works remotely and in the office for the ECM Department. This position is responsible for facilitating and assigning incoming referrals to ECM Enrollment.

QUALIFICATIONS

  1. High-School diploma or certificate holder in a healthcare field required.
  2. Excellent communication skills and ability to engage others.
  3. Proper phone etiquette.
  4. Experience in working with electronic healthcare systems, familiar with Microsoft Word Excel programs and developing reports.
  5. Strong organizational and follow up skills

ESSENTIAL DUTIES AND RESPONSIBILITIES

  1. Fields incoming calls and stamps it with the date received.
  2. Makes frequent outgoing calls to clients referred to us to determine eligibility and interest in services.
  3. Reviews referrals document for proper completion.
  4. Reviews and compares ECM Referrals according to the criteria for ECM services.
  5. Creates patient charts and uploads referral documentation into the EMR.
  6. Processes referrals and coordinates follow up activities for ECM Case Managers
  7. Confirms ECM enrollment and eligibility and communicates with Health Plans.
  8. Provides updates to inquiring parties regarding patient status and/or other relevant information.
  9. Tracks and ensures referrals are processed within the time frames stated in the ECM contract expectations.
  10. Reports any significant findings to the Program Manager as necessary.
  11. Conforms to all agency policies and procedures.
  12. Maintains/conserves confidentiality of patient and agency information at all times with HIPAA regulations.
  13. Regularly attends and participates in scheduled case conferences, staff meetings and agency in-services.
  14. Follows policies and procedures as per Employee Handbook.
  15. Maintains all required credentials up-to-date.
  16. Reports Fraud and Abuse.
  17. Knowledge of mandated reporting.
  18. Conducts timely recording and/or documentation of client contact.
  19. Attends Department meetings (in person, by phone or by web).
  20. Attends all State mandated in-service training.
  21. Performs other duties as assigned.
