Palliative Care Social Worker

  • Position: Palliative Care Social Worker
  • Pay Range: BSW $55,000- $60,000; MSW $60,000-$70,000
  • Reporting To: Regional Palliative Care Director
  • Work Type: Remote

POSITION SUMMARY

Libertana is a home health company based in Los Angeles with patients across California. We are seeking motivated and caring professionals to join our Palliative team and be a resource for patients in need. The Social Worker will participate in the interdisciplinary palliative care services team and provision of direct patient care and appropriate related services.

QUALIFICATIONS

  1. MSW preferred BSW required.
  2. Experience in a health care setting preferred.
  3. Active driver’s license

ESSENTIAL DUTIES AND RESPONSIBILITIES

  1. Provides psycho-social support to palliative care patients.
  2. Visit clients in their own homes on a regular basis based on the managed care plan.
  3. Uses their professional background and skills to assist the team and facilitate patient family meetings to create goals of care and the plan of care.
  4. Create a plan of care and implement services appropriately.
  5. Collaborate with the interdisciplinary team to create a plan of care.
  6. Assists the Palliative Care Coordinator in ensuring excellent patient care services.
  7. Coordinating educational and outreach services, leading quality improvement initiatives, and linking teams with outpatient and community services.
  8. Coordinates team efforts with appropriate hospital support departments to ensure smooth discharge planning and clear follow-up post discharge when a client is hospitalized.
  9. May assist in the development of creative community or system care options for patients.
  10. Connect family to resources and support within the community.
  11. Serve as a liaison within the school system for families in need.
  12. Serves as a liaison with the hospital hospice and other community hospices, home health, and nursing homes to ensure care coordination and appropriate assignments and resource allocation to meet patient needs throughout the continuum of care.
  13. Daily availability for the physician group and for social work support to provide updates on consultations.
  14. Participation in on-call coverage with the interdisciplinary team.
  15. Weekly attendance at interdisciplinary team meetings to review active cases.
  16. Attendance at monthly and periodic team meetings for the operational and administrative functions of the palliative care service.
  17. Ad hoc activities to support educational and outreach goals of the palliative care service.
  18. Community outreach to physicians, the public, healthcare facilities and other applicable organizations in the community.
  19. Participation in ongoing research and quality improvement projects on an as-needed basis.
  20. Other duties as assigned.
